Purpose

  • Begin/evolve a set of key criteria to inform development of person-focused questions regarding health

  • Revise/refine a framework for whole health approach to patient goal-oriented care based on what matters most

  • Consider/advance a set of potential questions to facilitate whole person care delivery for Medicare beneficiaries

  • 8 working groups focused on specific aspects of development and implementation
